      <description>&lt;p&gt;A lot of agent teams know they need escalation.&lt;/p&gt;&#xA;&lt;p&gt;Very few define it cleanly.&lt;/p&gt;&#xA;&lt;p&gt;So what happens in practice?&lt;/p&gt;&#xA;&lt;p&gt;The agent gets stuck, confidence drops, a customer asks something weird, a downstream system throws an error, or a policy edge case appears. Then everyone starts improvising:&lt;/p&gt;&#xA;&lt;ul&gt;&#xA;&lt;li&gt;should this go to support?&lt;/li&gt;&#xA;&lt;li&gt;should ops look at it first?&lt;/li&gt;&#xA;&lt;li&gt;is this a security issue or just a bad input?&lt;/li&gt;&#xA;&lt;li&gt;does finance need to approve this?&lt;/li&gt;&#xA;&lt;li&gt;should we pause the workflow or let it retry?&lt;/li&gt;&#xA;&lt;/ul&gt;&#xA;&lt;p&gt;That is not an operations model.&#xA;That is a coordination tax.&lt;/p&gt;</description>
